For chronic pain, first line evidence based medications include tricyclics, gabapentinoids and SNRIs. Treatment involves empirical, N-of-1 trials. Expect analgesic failure, but pursue analgesic success. There is a limited role for long term opioids. Analgesia is not normally distributed.
