Manual Muscle Testing
To isolate the flexor carpi ulnaris, examiners combine wrist flexion with ulnar deviation in a neutral or supinated arm position. This position biases the FCUโs line of pull and minimizes contribution from the flexor carpi radialis (which produces radial deviation). The patientโs forearm is usually supinated or in neutral, and the wrist is flexed toward the ulnar side against resistance. Palpation of the tendon proximal to the pisiform can help ensure FCU activation.
