Origin Humeral head origin is medial epicondyle from common flexor tendon. Ulnar head origin is medial aspect of the olecranon and proximal border of the ulna
Insertion Pisiform bone, hamate bone, and base of the fifth metacarpal
Action Wrist flexion, also ulnarly deviates the wrist/hand.

Manual Muscle Testing

To isolate the flexor carpi ulnaris, examiners combine wrist flexion with ulnar deviation in a neutral or supinated arm position. This position biases the FCUโ€™s line of pull and minimizes contribution from the flexor carpi radialis (which produces radial deviation). The patientโ€™s forearm is usually supinated or in neutral, and the wrist is flexed toward the ulnar side against resistance. Palpation of the tendon proximal to the pisiform can help ensure FCU activation.
